CCDF Certificate Program Re-Certification Forms

The Child Care Development Fund Certificate Program is a federally-funded service offered to eligible families with parents who are working or in school full time. Parents have the option to select a child care provider (state licensed or DES certified).
  • New Applicants: We are not accepting new applications at this time.
  • Re-Enrollment of Currently Enrolled Families: It's time to redetermine your eligibility for our program.  You are not required to complete a new application. The following forms must be received by our office no later than Oct. 31, 2018. Incomplete forms will be ineligible. You may fax the forms to 480-362-2243 or email Alizabeth.Philbreck@saltriverschools.org
    • Parent Responsibility Form
    • Re-enrollment Form
    • Documentation of all household income, including your two most recent pay stubs, as well as any court ordered child support, lease income, SSI, unemployment, financial aid award letter, insurance/annuity income, or TANF benefits.
    • Current class schedule (if you are using student status to meet eligibility requirements).
    • Protective care  court document, if applicable.

Your current certificate is valid until Dec. 31, 2018. If you are determined eligible for 2019, you will receive a new authorization effective Jan. 1, 2019 (valid through Dec. 31, 2019).
